The grant funded HIV Prevention Services Coordinator is responsible for facilitating St. Jude Neighborhood Health Centers successful participation in HRSA’s Ending the HIV Epidemic Initiative. Through effective project management support system changes that advance quality and value of HIV prevention within primary care.
Specifically, support the development and then the on-going strengthening of HIV prevention and care within our primary care setting. Plan, coordinate, and drive improvement in, HIV prevention & care along with overall grant project performance. Educate providers, caregivers and patients on issues related to HIV prevention and care.
This position is responsible for program logistics and communications and entails direct engagement with patients. Ensures completion of project deliverables all within the scope, timeline, budget identified. Position promotes cost effectiveness and operational efficiency in the oversight of the projects and communications that align to the mission and values of St. Jude Neighbor Health Centers.
Essential Functions (What You Do):
- In partnership with providers and operational leaders, develop workflows, protocols, standards of care, and formularies related to HIV prevention and care that result in highest-quartile quality outcomes and a high-level of patient satisfaction.
- Plan, Coordinate and report out SJNHC’s performance related to HRSA’s Ending the HIV Epidemic grant initiative. Engage and participate in all HRSA Ending the HIV Epidemic related meetings and trainings. Maintain a positive rapport with HRSA Ending the HIV Epidemic staff.
- Partner and strategize with HIV Prevention Team to screen and assess patients for HIV risk and in identifying HIV Prevention Intervention opportunities at the patient level.
- Develop and disseminate standardized patient and provider education materials related to HIV Prevention and treatment.
- Deliver HIV Prevention and Treatment education to caregivers, providers and patients.
- Monitor organizational performance on quality measures related to HIV Prevention and Care. Drive improvement in outcomes, where needed.
- Organizes, facilitates and documents internal Ending the HIV Epidemic project meetings.
- Prioritizes and organizes both short and long-term responsibilities in response to frequently changing demands and frequent interruptions.
- Identifies, documents, and monitors metrics of project performance; prepares presentations on program outcomes; and notifies stakeholders of problems affecting timelines and resources.
- Clearly and concisely informs project team members, SJNHC leadership, partners and stakeholders of project status, opportunities, and challenges on a routine (monthly) basis. Identifies and communicates early and often on issues needing escalation.
- Ensures and facilitates participation in project evaluation activities, including interviews and surveys, data collection, and site visits that meet stakeholder expectations.
- Directly administers HIV screenings, tests, and risk assessments to patients. Provide patients education on HIV prevention and treatment, resources, case management and referral to HIV prevention services and care.
- Develops and maintains positive and effective partnerships with Physician, HIV Prevention specialist, organizational leaders, and caregivers. Establishes and maintains a positive and effective partnership with Ryan White funded HIV treatment programs.
- Participate in and support other population and quality improvement opportunities and initiatives.

Pay Range: $31.47 to $41.72 Per Hour

Requirements
Required Education:
Master’s Degree in Health Education or Public Health
Required Experience:
- Minimum of two years’ experience with Health Education and/or project management.
Required Knowledge/Skills/Abilities:
- Demonstrate a commitment and passion for working with underserved patient populations.
- Computer proficiency in Word, Excel, and Visio.
- Demonstrates the ability to read, write and speak English and Spanish fluently.
Preferred Experience:
Experience using Epic Systems electronic medical record.
Preferred Knowledge/Skills/Abilities:
Knowledge of HIV prevention and treatment best practices.
