•      Bilingual (English/Spanish)

•      Committed to providing high-quality care to underserved patients in High Desert.

•      Willingness to work as part of a collaborative team that includes, social worker, substance use disorder counselor, and psychiatrist.

•     St. Jude Neighborhood Health Centers utilizes EPIC for our Electronic Medical Records (EMR) system.

•     Ambulatory (outpatient) 

•     Daytime work schedule - Monday through Friday 8AM - 5PM, potential option to work remotely one day per week.

Our health centers use a team-based model of care that includes family medicine, obstetrics, women’s health, oral and vision care, preventative and educational health, mental health, patient advocacy, case management, and community health. Most of our patients live below 200% of the Federal Poverty level, and are best served in Spanish. Essential Functions (What You Do)

· Complete psychosocial evaluations, assessments, and provide counseling services to a diverse population of patients with mild- to moderate- mental health diagnosis, including individuals with a history of substance use disorder. 

· Provide effective follow-up services and community referrals as appropriate.

· Provide crisis intervention to patients and their families.

· Initiate and complete all state-mandated documentation within specified time frames on child and elder abuse and domestic violence.

· Collaborate with medical providers, nurses, patients and their families to assess patients’ social needs and monitor progress of treatment.

· Ensure timely documentation of all patient contact in clear, concise and accurate notes with the patient’s electronic health record.

· Responsible for accurate and complete documentation of all patient care activities.

· Maintains a caseload; performs a variety of direct clinical, diagnostic, treatment, and outreach services.

  

Salary Range: $89,970 – $107,500 annually

Requirements

Work Environment & Physical Demands: 

The work environment characteristics described here are representative of those a caregiver encounters while performing the essential functions of this job.  

 Language Skills – Ability to read, analyze and interpret documents such as treatment records, test results, referrals, medical guidance, correspondence, protocols, and procedure manuals. Ability to write treatment notes and encounters. Ability to speak effectively to all team members and levels within the organization.  

 Reasoning & Mathematical skills – Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.  

 Physical Demands – Repetitive motions and routine use of standard office equipment such as medical devices, computers, telephones, copiers/scanners, filing cabinets. Ability to see, speak, walk, hear, stand, use of hand/fingers to handle or feel; climb stairs, stoop, carry/lifting up to 20 lbs. Prolonged periods of sitting at a desk, listening, and working on a computer. Frequent standing, and movement up and down in chairs. Ability to physically conduct all assessment and treatment measures of patients. Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception and the ability to adjust focus. The noise level in the work environment is usually mild. It includes open offices, office equipment, such as, phones, computers, and printer/copy machines, and waiting areas. The demands of the position include travel to assigned clinics and occasional organization-wide activities.  


Position Requirements

Education:
Master’s Degree in Social Work or Licensed Clinical Professional Counselor from an accredited University. 


License/ Certifications:
Current and valid license in good standing – Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) through the California Board of Behavioral Sciences.  

Provider level basic life support (BLS) from AHA approved program. 


Experience:
Two years' recent experience, preferrable in a medical setting. 

 

Knowledge/Skills/Abilities: 

  • Office administrative practices and procedures, including EMR (preferably EPIC), management and the operation of standard office equipment. 
  • Familiar with the culture and customs of the clients with whom they will interact.  
  • Communication skills consistent with the facilitation of mental health wellness topics to individuals, small and large groups of community residents.  
  • Must be able to demonstrate the      ability to read, write and speak English and Spanish without an interpreter.
  • Demonstrates interpersonal/cultural sensitivity and work with a diverse and often underserved population.  
  • Effective written and verbal communication across all levels of the organization and with vendors.
  • Highly organized with strong time management skills, being able to perform. simultaneous tasks, prioritize them effectively and follow through quickly. 
  • Proficiency with computer software applications (e.g., Microsoft Word, Excel, Outlook, PowerPoint; Internet) 

Preferred Requirements

License/ Certifications:
Member of NASW; CAMFT 


Experience:
Experience in FQHC or Community Health Setting 

Experience in Medical Behavioral Health Model of Practice
