What are the responsibilities and job description for the Pregnancy Support Navigator position at St. Luke's Cornwall Hospital?
Rate Range: $22.00 - $25.00 per hour
Position Summary: The incumbent in this position, in collaboration with the Director of Obstetrics is responsible for guiding the obstetric patient and her family through the continuum of care, from pregnancy through birth and the post-partum period in an efficient and well-organized manner. This role serves as a link between the hospital, the patient and the OB, any Medical, Clinics and Pediatric offices in connecting with services surrounding the patient experience. The purpose of this role is to focus on building positive, ongoing relationships with doctors and patients and their families to ensure the birthing experience is positive. This navigator will perform outreach by acting as a liaison with outside agencies, perform on-site visits and community outreach through presentations. This person will create relationships with pregnant women who suffer from Substance Use Disorder (SUD). They will guide the mother into a program (if not already in a program) and support the mother through her pregnancy, delivery, infant's NICU stay and post-partum to ensure she has the support to remain free of illicit drugs and help to prepare the new mother to care for her infant in a safe environment. Weekly support groups will be held and coordinated by this navigator.

Licensure, Registration or Certification: Substance Abuse Counselor Certification (CASAC) preferred.

Education/Training
Position Qualifications
- High School Diploma or equivalent required. Business School Certificate or Associate Degree preferred.
- Ability to read, write and communicate in English.
