  • JOB SUMMARY
    • As an advocate for the patient, the Behavioral Coordinator assists patients with mental health and substance abuse needs to reach optimal function and health in an appropriate environment that would support their needs.

      The Behavioral Health Coordinator applies the process of assessment, planning, implementation, monitoring, evaluation and coordination of care to meet the patient's health care needs across the continuum in collaboration with health care team members to ensure safe, quality and cost effective care. The individual will attempt to resolve social and environmental difficulties interfering with the patients’ ability to maximally manage their mental health and overall quality of life. The Behavioral Health Coordinator will triage referrals from primary care providers to the appropriate behavioral health provider and service.

      As a member of the multidisciplinary team, the Behavioral Health Coordinator works closely with physicians, financial staff, and ancillary staff to facilitate those functions associated with arranging care and providing referrals.

      The Behavioral Health Coordinator will possess significant independent judgment, analytical, problem solving, collaborative skills in order to organize and prioritize multiple tasks to provide patient centered leadership and direction.

  • MINIMUM QUALIFICATIONS
    • Education / Experience:  Master’s Degree in Social Work, Psychology, Criminal Justice or closely related area of study OR Bachelor’s Degree in Social Work, Psychology, or closely related area of study or a closely related degree and three (3) years’ experience in behavioral health or case management.

  • KNOWLEDGE, SKILLS AND ABILITIES
    • Must demonstrate the ability to communicate effectively, understand mental status concepts, symptomology, treatment and service terminology and apply each of these concepts, and identify, implement and coordinate the services and supports identified in the treatment plan.
    • Possesses planning and organizational skills and expert skills in prioritization.
    • Understands the nature of social diversity and oppression with respect to race, ethnicity, national origin, color, sex, sexual orientation, gender identity or expression, age, marital status, political belief, religion, immigration status, and mental or physical disability.
    • Keep current with government regulations.
    • Annual competencies as required by Aspirus and/or various regulatory agencies based on entity and/or job position.

    • READING - Intermediate: Ability to read and interpret documents such as operating and maintenance instructions and procedure manuals.
    • WRITING - Intermediate: Ability to write routine reports, correspondence, or procedures
    • SPEAKING - Intermediate: Ability to effectively present information in one-on-one, small group situations or before groups of customers, clients, and other employees of the organization.
    • MATHEMATICAL SKILLS - Intermediate Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals and to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to compute rate, ratio, and percent and to draw or interpret bar graphs to apply concepts of basic algebra and geometry.
    • REASONING ABILITY - Intermediate Skills: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
    • AGE SPECIFIC COMPETENCIES - Skilled at assessment and knowledgeable of growth and development. Provides appropriate care respective to the ages of the patients served per clinical environment.

      • Regularly exposed to the risk of blood borne diseases; contact with patients under wide variety of circumstances; exposed to unpleasant elements (accidents, injuries and illness); subject to varying and unpredictable situations; handles emergency or crisis situations; occasionally subjected to irregular hours
