Position Summary:
The role of the Clinical Referral Specialist is to act as the in-office clinical liaison that represents the agency via telephone for referrals for hospice and home health, particularly for those patients with complex needs. Responsible for promoting teamwork with all members of the healthcare team. Performs duties in a manner consistent with St. Luke’s mission and values.
Education, Experience, & Licensing Requirements:
Education: Graduate of an accredited nursing school.
Experience: RN: Minimum of two years recent clinical experience required, with a minimum of one year home care and/or hospice experience.
LPN/LVN: Minimum of three years clinical experience required, with a minimum of two years home care and/or hospice experience.
Licensure: Must have a current, valid clinical (RN, LPN/LVN) license to practice nursing in the state of Missouri
