Position: 2nd Shift Nurse (RN/LPN)
Job Description
St. Luke's Hospital Transitional Care Center is currently seeking a compassionate and skilled 2nd Shift Nurse (RN/LPN) to join our team. The ideal candidate will have a strong passion for providing exceptional care to patients in a transitional care setting. This position is specifically for Wednesday & Thursday evenings, providing care and support to patients during the second shift of the day.
Responsibilities
- Assessing and evaluating patient health statuses
- Developing and implementing individualized care plans
- Administering medications and treatments
- Monitoring and documenting patient progress
- Collaborating with healthcare team members
Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Iowa
- Previous experience in a transitional care or similar setting
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work on Wednesday & Thursday evenings
