JOB SUMMARY

The Clinical Coordinator is responsible for maintaining clinical practice standards, utilizing and promoting evidence based practice and guiding the VNA interdisciplinary team. Acts as a staff resource and liaison to facilitate staff development. Works collaboratively with Patient Care Manager to maintain standards of professional nursing practice.

WORK PERFORMED

JOB DUTIES AND RESPONSIBILITIES :

ESSENTIAL FUNCTIONS :

  • Demonstrates expertise in areas of clinical practice.
  • Supports Patient care Manager in maintaining and supervising daily operations, clinical oversight, training and competency.
  • Assists Patient Care Manager in developing, evaluating and adjusting current and future staffing based upon patient care needs; initiates adjustments as needed.
  • Uses Nursing Process as framework for patient care delivery.
  • Acts as a liaison between interdisciplinary team to facilitate optimal case management and discharge planning.
  • Assigns responsibility for patient care.
  • Assists with staff education activities (i.e. orientation, competency, skill reviews, mandatory in servicing, and evaluation of student clinical placements.)
  • Provides input to annual performance reviews of assigned staff.
  • Maintains awareness of unit budget.
  • Consistently fosters a positive work environment.
  • Takes active role in program performance improvement initiatives.
  • Maintains responsibility for own professional development through active participation in in-services and / or continuing education programs.
  • Participates in identifying departmental needs and supports departmental goals and objectives that contribute to the growth of the Program.
  • Functions as a professional role model and resource person providing guidance to co-workers.
  • Complies with established chain of command for work-related problems.
  • Assists with maintenance of departmental records for administrative and regulatory issues.
  • Maintains confidentiality of all materials handled within the Network / Entity as well as the proper release of information
  • Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.
  • Demonstrates / models the Network’s Service Excellence Standards of Performance in interactions with all customers (internal and external).
  • Demonstrates Performance Improvement in the following areas as appropriate : Clinical Care / Outcomes, Customer / Service Improvement, Operational System / Process, and Safety.
  • Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.
  • Complies with Network and departmental policies regarding attendance and dress code.
  • Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated

  • Stooping (bending at the waist) and crouching (stooping and bending at the knees) – required frequently
  • Handling (grasping) – required frequently
  • Fingering (fine manipulation) – required only occasionally
  • Important : The functional capacity to perform heavy work includes the functional capacity to perform medium, light and sedentary work.

    Good physical and mental health.

    Neat, clean, free from body odors, conforms to the uniform code.

    Constant standing and walking during work periods.

    Turning, stooping, bending, climbing, stretching and lifting in the provision of nursing care

    Finger and hand dexterity necessary to handle delicate instruments and other equipment

    Visual and auditory acuity required to provide comprehensive nursing care.

    Environmental Conditions :

    Inside – Office environment and patient homes

    Outside - Traveling to cases and / or clinics in extreme cold and heat,

    wet and / or humid conditions, snow, ice and / or fog

    Hazards - Exposure to contagious disease, road and driving, domestic pets.

    POTENTIAL ON-THE-JOB RISKS :

    Employees who work in patient care areas where they may have potential exposure to patients with suspected or proven tuberculosis (TB) must have the ability to wear a particulate respirator and be fit-tested in compliance with the current recommendations from the CDC (enforced by OSHA) or must provide physician documentation as to the inability to wear a particulate respirator.

    SPECIFIC PROTECTIVE EQUIPMENT AVAILABLE :

    Gloves, gown, mask, goggles

    MOST COMPLEX DUTY :

  • Coordination of care utilizing critical thinking skills .
  • SUPERVISION (RECEIVED BY AND / OR GIVEN TO) :

    Supervised by the Patient Care Manager

    Provides supervisions for the department based team.

    COMMUNICATIONS :

    Must be able to communicate with other departments, patients, families, the community and medical staff.

    QUALIFICATIONS

    MINIMUM)

    EDUCATION :

    Graduate of an accredited professional nursing program.

    Registered Nurse with current license to practice in the state of Pennsylvania or seeking licensure through endorsement.

    TRAINING AND EXPERIENCE :

    Two years experience in home health or hospice.

    Evidence of successful completion of BLS.

    WORK SCHEDULE :

    12hr day-shift, Saturdays and Sundays.
