What are the responsibilities and job description for the Case Manager-RN position at St Lukes Cornwall Hospital Health System?
Salary Range: $103,496 - $114,031 based on experience
Applies clinical experience to provide clinical reviews to the insurance carrier, utilizing nationally recognized criteria in review development. Works with the health care team to ensure documentation supports level of care needed. Coordinates care to prevent delays in care. Performs discharge planning for those requiring home care.
Qualifications: Graduate of NLN Accredited School of Nursing, BSN preferred. Ability to read, write and communicate in English. Minimum of one (1) year experience in an acute care hospital. Strong clinical, organizational and interpersonal skills. Current New York State Licensure as a Registered Professional Nurse. Certification in Case Management required 18 months after eligibility. PRI and Screeen Certification required within 3 months of hire.
