Healthcare Coordinator

Pay Rate: $22-$26 per hour

Responsible for supporting patient care, ensuring safety protocols are followed, improving performance, assisting in the continuity of care, and facilitating communication between patients and healthcare providers. This role requires attention to detail, strong communication skills, and a compassionate approach to patient care, with a focus on enhancing patient outcomes through careful management of care transitions, patient education, and collaborative teamwork. This position has responsibilities on both campuses and requires heavy involvement with the community.

Position Qualifications :

1. Education/Training : High school diploma or equivalent required. Associate's degree in a healthcare-related field preferred. Ability to read, write, and communicate in English.

1. Experience : At least three (3) years of experience in direct patient care., Experience in social service delivery with elderly and/ or family populations is preferred. Knowledge of community supportive services in the area (Area on Aging, Meals on Wheels, transportation, etc.). Basic understanding of medical terminology is required. Microsoft Word and Epic experience is required. Microsoft Excel and PowerPoint experience preferred.

1. Licensure/Certification: Medical assistant certification from an accredited program (e.g., CMA, RMA) preferred. BLS certification from American Heart Association required
