Qualifications and description for a Social Worker, LSW/CSW:
- NJ Social worker, CSW/LSW License
- Conduct Social Service assesments and documents according to state and federal regulations
- provide Physco-social assesment and relevant parts of Minimum Data Set (MDS)
- Counseling Residents
