Formal education required:
Licensed and certified as a physical therapy assistant by New York State.
Graduate of a two-year, college-level program approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association.
Previous Experience Required
Minimum of two years of physical therapy assistance.

Job Duties / Responsibilities
- Implements physical therapy services in accordance with the plan of treatment planned, delegated, and supervised by the PT and signed by the physician.
- Assists the PT in assessing the patient's functional level by applying appropriate tests. Assists in the development and revision of the plan of care in accordance with the supervising PT.
- Treats patients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care.
- Observes, records and reports the patient's reaction to treatment and any change in the patient's condition to the physician and/or PT and other staff involved.
- Instructs and advises patients, family and other health team personnel including, when appropriate, home health aides, in certain phases of physical therapy in which they may assist the patient; also instructs all of the above in the use of prosthetic and orthotic devices and durable medical equipment, if appropriate.
- Assists in preparing patient clinical notes and progress reports according to Home Care policy and time frames within the electronic medical record (EMR).
- Attends patient care conferences. Initiates patient care conferences for complex and/or multi-disciplinary patients when needed or helpful to ensure good coordination of care.
- Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation within designated time frame.
Formal education required:
