Additional Details

American Heart Association certification is required for all direct patient care personnel.

Must include an image of the front and back of the certification card.

Verification of licenses or certifications must be performed within past 30 days.

Upload state verification if available with NURSYS for RNs and LPNs.

Must have page 27 signed form on file in Trinity VMS.

Must have signed Trinity Health Confidentiality and Information System Security Agreement.
