What are the responsibilities and job description for the Case Management Supervisor position at St. Nicks Alliance?
The Case Management Supervisor, under the direct supervision of the Assistant Director of Clinical Services (Clinical Supervisor) will ensure staff are providing quality intensive case management for individuals and families living in supportive housing. Utilizing evidence-based practices the position will promote and model best practices for social services staff, assist in coordination of mental health and community-based services and increase the ability of staff to address behavioral health issues in a culturally-competent, strengths-based manner. The Case Management Supervisor is responsible for ensuring trauma-informed crisis intervention and management.
Responsibilities:
- Supervises the Case Managers and Peer Specialists to ensure they are providing excellent comprehensive case management for consumers including assessments, person-centered service plans, case coordination and evaluations.
- Ensures consumers are referred and engaged with medical care, mental health treatment and substance use programs as needed.
- Completes annual mental health assessment and substance use assessments.
- Coordinates Case Manager caseloads and reviews all records for compliance with HRA standards to ensure charts are audit ready at all times.
- Ensure 100% of new residents have an initial assessment within 30 days of enrollment.
- Ensures staff make appropriate referrals to mental health providers, medical care, social service agencies, substance abuse programs, legal services, consumer advocacy and empowerment programs and health and wellness agencies.
- Supervises staff in monitoring consumer’s health and functional status in order to make referrals for housekeeping and homecare services, as appropriate.
- Ensures Case Managers secure all needed benefits and entitlements for consumers
- Communicates and collaborates with service providers to enhance engagement and ensure coordination of care.
- Supervises staff to ensure trauma informed approaches to crisis intervention including de-escalation, risk assessment, safety planning and incident reporting and management.
- Monitors and coordinates outreach to crisis services including Mobile crisis, ACT teams, Adult Protective Services and mental health in-patient services.
- Provides training to staff, in evidence-based approaches and practices including trauma-informed care, recovery-oriented practices, harm reduction, person-centered planning, motivational interviewing and health advocacy.
- Strengthens Case Manager’s clinical skills, understanding of HIV/AIDS, and knowledge of benefits and referral sources through on-going weekly individual and group supervision.
- Provides case management to individual consumers, as needed.
- Performs other duties as requested by Deputy Director for Supportive Housing, Deputy Director for Social Services or the Executive Director.
Qualifications:
- Required MSW ; Preferred LMSW or LMHC
- Must have experience in case management and service planning with those with mental illness, substance use disorder or HIV/AIDS.
- English/Spanish preferred but not required.
- Knowledge of HIV disease progression; emotional/behavioral health; parent-child relationship dynamics; and diagnostic classification.
- Strong expertise in strengths-based, solution-focused, and person-centered practice
- Exceptional writing and oral communication skills
- Experience with harm reduction, motivational interviewing, recovery-oriented practice, trauma-informed care and other evidence-based practices
