Requirements: Bachelor's degree in Social Work or Human Services Field
Preferred: training or experience in Geriatrics

Resident and Family Support
- Aids and welcomes new residents to the facility with visits, support, and counseling (resident and family).
- Meets with family members and residents at the time of admission to explain the Patient's Bill of Rights, Admission forms, Advance Directives, Durable Power of Attorney forms, initiating telephone service, completing medical assistance, SS subsidy and PACE applications, and answers questions on Medicare B determination and Medicare D.
- Assists with problem-solving.
- Serves as a patient advocate with staff, families, and administration, communicating residents' needs.
Admissions and Discharges
- Assists admissions coordinator as needed to explain services to prospective residents and families, provides tours, assists with the completion of admission applications, explains policies and procedures (i.e., Medicare/Medicaid, etc.), and assists with room moves as needed.
- Mails information regarding services, including adult day care, respite care, support group, etc., to prospective residents/families.
- Discusses discharge plans and assists with discharge arrangements (i.e., home health services, meal services, communication, etc.).
- Reviews or completes PASSRs upon admission/readmission, updating as required. Completes Medical Assistance 408s for targeted residents.
Documentation and Assessment
- Collects information via interviews with residents, family, and friends and through clinical records to prepare social history, social evaluation, discharge plan, progress notes, and other documentation on a timely basis.
- Documents initial assessment and social history within 48 hours after admission and assessment within 14 days of admission. Completes assigned areas of MDS 2.0 for initial, quarterly, and significant change assessments and completes relevant tracking forms. Must complete within 7 days of the event (i.e., discharges, re-entry, or MA status change).
- Formulates individualized goals and approaches for each resident, documenting progress. Participates in the completion of a comprehensive care plan with the interdisciplinary team within 7 days of the comprehensive assessment.
- Keeps files of care plan letters and gives them to medical records annually or when charts are closed.
- Documents specific recommendations to assist residents in care and emotional welfare. Prepares written discharge plans and updates at required intervals.
Care Planning and Team Collaboration
- Actively participates in regularly scheduled care plan conferences with the interdisciplinary team. Assesses residents' current conditions and needs, formulates social goals for each resident’s plan of care, and reviews/updates at required intervals.
- Works with all departments to provide quality care to each resident. Participates in family conferences as needed.
- Serves as an advisor for applicable in-house meetings (i.e., resident council, behavior management, PEER). Refers suggestions/concerns to the proper department.

Pay: From $20.95 per hour
