The Community Hospice has a wonderful opportunity for an RN to work as part of our Hospice Care Team caring for patients and families in the homecare and NH setting. Primary region to cover is Schenectady, Albany and Rensselaer counties.
Work only Every Saturday&Sunday 8a-8p - no hours required during the week once trained

- Provide direct oversight and guidance for the Anticoagulation Clinic Services.
- Perform drug regimen / laboratory data review and adjusts anticoagulation therapy, when appropriate, to maximize outcomes for all Anticoagulation Clinic Service patients.
- Provide appropriate patient monitoring to ensure that desired outcomes are met by following Anticoagulation Clinic protocols and established best practice guidelines.
- Observe, record and report patient’s condition and reaction to drugs and treatments to physicians. Prescribe medications as outlined in provider approved CDTM.
- Oversees appointment bookings and ensures preferences are given to patients in emergency situations. Maintain timely flow of patients.
- Respond to and refer incoming telephone calls (Triages). Instructs patient and family regarding medications and treatment instructions as directed by physician.
- Educate patients and families regarding anticoagulation therapy.
- Maintains organized documentation on recommendations, interventions, follow-up actions and outcomes.
- Communicate effectively with prescribers referring patients to the Anticoagulation Clinic.
- Maintain competency in anticoagulation care. Serve as an active patient advocate regarding anticoagulation therapy. Use a team approach to working with staff and others
- Active Participation in the training of St Peters PGY-1 Pharmacy residents and PharmD students
- Participates in performance improvement initiatives and research as appropriate.
- Performs other related duties as assigned such as quality assurance reports, preparation for regulatory inspections, etc.
