- Provides discharge planning as a member of an interdisciplinary team.
- Provides discharge planning, referrals, coordination with community providers and provision of other treatment related duties.
- Responsible for patient progression of care, interviewing patients for initial discharge plans and coordinating care to the next appropriate site of care.
- Bachelor's degree in related field required, or associate degree in related field with QHP required.
- Experience in a healthcare/medical setting, preferred.
- Strong writing and communication skills are required.
- Experience working with a diverse population and a strong understanding of multicultural issues is preferred.
