Registered Nurse-Liaison-Hospice-Intake-Saratoga County
The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as a Liaison in local hospitals in Saratoga County.

What you will do:
- Evaluates patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care.
- Coordinates with the patient’s community physician and our hospice medical directors to confirm eligibility for hospice services.
- Gathers information and ensures a smooth handoff to the hospice RN admitting the patient/family to services.
- Admits patients to Hospice in the hospital setting upon eligibility.
- Works closely with the hospital case management department.
- The work schedule is flexible, generally M-F weekdays 8am to 4:30pm .
What you will need:
- A current license to practice as a Registered Nurse in the State of New York
- Associates Degree in Nursing, BSN preferred
- 1-2 years recent acute care experience. Hospice or RN Liaison experience a plus but not required. 1 year experience as an RN , preferably in Hospice/Home Care or acute care.
- The RN must be able to communicate effectively, verbally and written
- Valid Driver's License and Reliable vehicle
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
- Understanding of Hospice principles preferred
Training provided in end of life care.

Pay Range: $35.00-$47.75 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
