RN Intake - Per Diem - Eddy SeniorCare (Latham)
Eddy SeniorCare has a rewarding opportunity for an RN to work out of our 385 Watervliet Shaker Road, Latham location!
PACE is a unique program on the forefront of trends in healthcare, combining an insurance role (we are the Medicare / Medicaid insurer for our participants) with a comprehensive healthcare provider role (primary care, home health, adult daycare, transportation, all coordinated by an interdisciplinary team).
This position is a community based RN role responsible for the following :
- Initial intake, 6 months and annual UAS assessments of all enrolled PACE participants
- RN assessment to include UAS, determination of support needs and collection of information to assist in determination if the participant continues to meet eligibility criteria
- Identifies risks for avoidable ER and hospital utilization and communicates promptly to the RN Case Manager and PACE primary care provider.
- Updates RN Case Manager regarding changes in participant needs in conformance with Agency policies and procedures, State and Federal regulations, and professional, legal and accrediting body standards of practice.
- Assists the patient in achieving his / her optimal level of health and function.
- Assesses, plans, implements and evaluates patient care using the nursing process.

Pay Range : $36.50 - $49.95
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
