Social Worker Palliative Care – Samaritan Hospital (Part-Time)
Position Overview:
Palliative Care Partners is seeking a dedicated Part-time Social Worker to join the inpatient palliative care team at Samaritan Hospital. The role involves assessing and addressing the psychosocial needs of patients with serious illness, providing therapeutic interventions, and contributing to the development of collaborative care plans.
Key Responsibilities:
- Conduct psychosocial-spiritual assessments and provide therapeutic interventions to patients and families.
- Facilitate patient-family meetings for goal setting and advanced care planning.
- Participate in daily interdisciplinary rounds to assess complex cases.
- Collaborate with healthcare team members, including Physicians, Nurse Practitioners, and other staff, in creating and coordinating care plans.
- Serve as a liaison between inpatient and outpatient palliative care teams, hospice, and community services.
Qualifications:
- Master’s in Social Work (MSW) with LMSW/LCSW or LMHC certification required.
- Minimum of 3 years of clinical experience in a medical, psychiatric, or healthcare setting; 2 years post-MSW preferred.
- Strong knowledge of healthcare delivery systems, community resources, and at-risk populations.
- Leadership experience in team settings or project development preferred.
- Supervisory experience is a plus.
Compensation:
- Pay range: $31.50 - $45.23 per hour, based on experience, skills, and education.
