Job Requirements

  • Attach scanned copy of BLS certification to candidate's record in TrinityVMS
  • State License / Certificate Verification must be within 30 days of start
  • Fit Test required for onboarding
  • Trinity 9 Panel Drug Screen required before start
  • Sign and file Trinity Code of Conduct Form
  • Complete standard i9
  • Sign and file Trinity Confidentiality of Patient and Proprietary Information and Information System Security Agreement
  • Complete Trinity Integrity and Audit Services Form
  • Complete Trinity Specialty Specific Competency Testing within a year
  • Complete Trinity Color Vision Test
  • COVID-19 Vaccine required, with proof of booster if applicable
  • Provide proof of MMR immunization or primary source IgG titer
  • Complete Hepatitis B vaccination series and provide documentation
  • Provide Influenza Vaccine documentation, declination not accepted for certain states
  • Provide proof of Varicella immunization or primary source IgG titer
  • Upload current, non-expired Tdap vaccine or declination if applicable
  • Attach proof of recent Physical / Health Statement
  • Submit Resume, Skills Checklist, and 2 Professional References
  • Sign and file Trinity Conflict Of Interest Disclosure Form
  • Complete Trinity Criminal Background Check within 30 days of start
  • Verify and upload highest level of academic education or certifications if required
  • Provide EPLS report from the website or third party dated within 30 days
  • Provide scanned OIG result print-out or third party result dated within 30 days
  • Complete and attach the NSO report within 30 days of start
  • Fill and upload Trinity System Access Form one week before start
  • Complete 2 Step PPD OR IGRA (Quantiferon) for TB
  • Complete Trinity OFAC within 30 days of start
