Responsibilities

- Provide ongoing case management services, including regular follow-ups and progress evaluations.
- Facilitate access to community resources such as Section 8 housing assistance, mental health services, and employment programs.
- Implement crisis intervention strategies to support clients during critical situations.
- Utilize motivational interviewing techniques to encourage client engagement and self-sufficiency.
- Collaborate with multidisciplinary teams to coordinate care and ensure comprehensive support for clients.
- Maintain accurate documentation of client interactions, progress notes, and service outcomes in compliance with organizational policies.
- Stay informed about best practices in social work, behavioral health, and workforce development.
Qualifications
- Bachelor’s degree in Social Work or a related field; Master’s degree preferred.
- Proven experience in case management or social services with a focus on crisis management and behavioral health.
- Familiarity with Section 8 housing programs and workforce development initiatives.
- Strong skills in motivational interviewing and addiction counseling are highly desirable.
- Excellent communication and interpersonal skills to build rapport with clients from diverse backgrounds.
- Ability to work independently as well as collaboratively within a team environment.
