JOB DESCRIPTION AND POSITION REQUIREMENTS
Work Shift: Mon - Fri
JOB SUMMARY:
The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and helps coordinate a plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members.
MINIMUM QUALIFICATIONS:
- LMSW required – must be under supervision for LCSW; LCSW preferred.
- Working knowledge/experience in case management, payors and reimbursement.
- Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.
- Organized and efficient in establishing priorities and performing tasks within specific timeframes.
- Self-motivated requiring minimal supervision.
- Views problems as a challenge and investigates alternatives which demand creativity, flexibility and the ability to work under stress.
- Responsible for maintaining strict confidentiality of all verbal & written information and documents. Familiarity with and appropriate use of community resources.
Preferred Qualifications: LCSW preferred. Experience in psychosocial and therapeutic counseling preferred.
PHYSICAL DEMANDS:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 50 pounds is required.
