JOB DESCRIPTION AND POSITION REQUIREMENTS
Work Shift: PRN (as needed) 8am - 6:30pm (10-hour shifts) Mon - Fri
Job Summary:
Responsible for managing patient populations with acute, chronic illness and/or multiple co-morbidities, identified as requiring additional transition of care support following or during a visit to the Emergency Department setting. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the ED Navigator assists patients to gain self-efficacy/management skills, achieve optimum functional health status, and quality of life. The ED Navigator assists patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes.
Minimum Qualifications:
Licensed Practical Nurse (LPN), Respiratory Therapy, or Rehab Services. Must have a clinical background with strong problem solving skills. Two years of Acute Care Hospital, Emergency Department, Care Coordination, and/or Utilization Management experience preferred. Bachelor’s Degree preferred. Maintain licensure required for specific field.
Demonstrates exceptional communication and negotiation skills. Demonstrates organizational, prioritization and time management skills. Self-motivated requiring minimal supervision and ability to work independently. Demonstrates leadership skills. Strong interview, assessment and problem solving skills. Maintains clinical and professional case management expertise. Computer skills in Word, Excel, MS Office, Outlook, Electronic Health Record.
Physical Demands:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 20 pounds is required. Must be able to work with a moderate level of noise.

Constant (67%-100%) - seeing
Frequently (34%-66%) - lifting, carrying, pushing/pulling, reaching, handling/feeling, talking, hearing
Occasionally (1%-33%) - climbing (stairs, ladders, etc.), stooping, crouching
