The Supportive Housing Case Manager (CM) will provide support services to tenants in a congregate model of permanent supportive housing at Liberty Commons in Middletown. The population of tenants will include adults with disabling conditions such as mental illness, substance abuse, HIV/AIDS and homelessness. The Housing Case Manager reports to the Director of Housing & Community Services.
JOB RESPONSIBILITIES:
1. Welcome new tenants and provide general orientation to Liberty Commons, case management services and neighborhood/community resources.
2. Intake/Assessment of all participants within thirty days of admission to program; update needs assessments every six months thereafter.
3. Develop service plan within sixty days of admission to program and every six months thereafter.
4. Provide information about (and access to) food and other community resources, mental health and substance abuse treatment, harm reduction, medical attention, employment, and legal support.
5. Help with financial matters, accessing entitlements, planning social activities, providing transportation; assist with activities of daily living, apartment upkeep and being a good neighbor.
6. Organize and participate in on- and off-site social activities with tenants; provide support to tenant organization as requested.
7. Provide informal counseling and mediation to help resolve behavioral problems, emergency situations, potential lease violations and/or disputes with property management.
8. Advocate for tenants in housing and in the community; be available to help access appropriate resources as requested by tenant.
9. Maintain contact and support with tenants when they enter long-term treatment facilities, hospitals, or correctional system.
10. Maintain a good working knowledge of and relationships with other service providers and resources in the community for mental health and substance abuse treatment, medical attention, employment, entitlements and benefits, income, financial management, social activities, transportation, legal support, and ADL’s.
11. Referrals (and follow-up) to other community providers will be made as appropriate.
12. Maintain detailed and ongoing progress notes for each tenant and include in charts all information pertinent to their case management per supportive housing standards and best practices; including but not limited to emergency forms, referral forms, releases of information, and cash flow charts.
13. Log daily activity of client contacts (face-to-face, collateral and staff) as required by DMHAS contract.
14. Participate in staff meetings and trainings as required.
15. Maintain confidentiality.
16. Serve as a flexible team member to provide additional services as needed.
PREFERRED QUALIFICATIONS: B.A. degree, and/or min. 4 yrs. experience in human services; knowledge, interest, & some experience in services/issues related to homelessness, substance abuse, and mental illness. Willingness to use harm reduction strategies. Fluency in Spanish and English desirable.
HOURS: 35 hours per week, M-F (day shift, input accepted to set schedule), flexible as needed by caseload and activities.
