SUMMARY
The Rapid Rehousing (RRH) Case Manager functions as an integral member of the SVDP team, reflecting the essential value that youth, peer and ally partnerships provide the organization with its strength and uniqueness. The RRH Case Manager works with youth age 18-24 to ensure their duties are in alignment with the values and mission of SVDP. Service model includes Housing First, Trauma Informed Care, Hard Reduction, and Motivational Interviewing. Case Manager provides direct youth-centered and strengths-based services to youth experiencing homelessness. Services include encouragement of engagement of youth, supporting youth to accomplish their goals around achieving stability, gains a deep understanding of available local resources to reduce and remove barriers to housing and advocates for systems change.
PRIMARY DUTIES
- Providing youth flexible, responsive case management that addresses their variety of needs including obtaining identification; accessing public benefits; connecting to health, mental health, and substance abuse care; accessing childcare; connection to employment navigator program and other supports
- Building authentic, caring and affirming relationship with youth
- Supporting youth in identifying their strengths and use them to set goals together
- Working in strong collaboration with the Housing Services Team
- Monitors and provides linkages for participants in the areas of health, mental health, education and employment, housing, mainstream benefits, and parenting assistance.
- Regular case management to youth in the community once a week at youths home, job and other locations as needed.
- Intensive, youth-centered budgeting and financial goal setting
- Entering youth data into the HMIS system and Salesforce
SECONDARY DUTIES
- Keeps updated on most effective means of accessing services available within the larger community and maintains an active data base for referrals and community partners.
- Ensures that each participant agrees, understands and signs and follows the Participant Service Agreement.
- Provides crisis intervention as and when needed.
ADDITIONAL DUTIES
- Assists the Program Coordinator in representing the mission and interest of Youth Rapid Rehousing in accordance with St. Vincent de Paul of Baltimore to various groups, organizations, and service providers.
- Assists with and attends special events held by the Center and St. Vincent de Paul as necessary.
- Upholds the vision, mission and values of St. Vincent de Paul.
- Attends all staff meetings and other meetings as directed.
- Supports a welcoming and positive environment for volunteers and works with them effectively.
- Other duties as assigned within the scope of the position.
QUALIFICATIONS
- A Bachelors degree in human services, social work or a closely related field of related experience.
- Minimum of two (2) years successful in working with youth who are homeless and/or materially poor in an urban setting.
- Knowledge of and respect for confidentiality.
- Regular access to a car for community-based case management services.
- Desire to support homeless young adults in a fast-paced environment.
- Desire and ability to work as a member of a team.
- Welcoming demeanor and strong oral and written communication skills

Pay: $54,000.00 - $57,000.00 per year
