What are the responsibilities and job description for the Homeless Shelter Services: Case Manager position at St. Vincent de Paul Society of Lane County?
1. PRIMARY PURPOSE -
The purpose of this role is to help clients access permanent housing and gain stability. This is an in-person position and telework/remote work is not available.
2. ESSENTIAL DUTIES AND RESPONSIBILITIES -
- Utilizing internal tools and assessments and Lane County’s Homeless Management Information System (HMIS), collect and input client-level data for tracking and reporting purposes.
- Maintain and support caseload of clients who are working towards a goal of self-sufficiency and securing permanent housing.
- Participate in case-conferencing opportunities and attend meetings, as available, in order to enhance support and coordinated care for participants.
- Support program participants in accessing services to help increase their overall stability, basic needs, safety, income, health and wellness, support networks and community connections.
- Support program participants in overcoming housing barriers.
- Help participants gather and store the documents they need for public benefits, employment, and housing opportunities.
- Help participants create employment/tenant resumes and key information to use on employment/housing applications.
- Support program participants in searching for and accessing employment/housing.
- Build relationships and rapport with community partners, property management agencies and local housing providers.
- Participate in case-conferencing opportunities and attend meetings, as available, in order to enhance support and coordinated care for participants.
- Ensure confidentiality and maintain up-to-date and accurate client records.
- Transport clients to and from community resources and assist them in accessing community resources.

- Communicate effectively and respectfully within the context of varying beliefs, behaviors, orientations, identities, cultural backgrounds regarding clients, staff and community partners.
- Maintain professionalism, safe, respectful and productive environment for clients, staff and volunteers.
- Utilize de-escalation and trauma-informed approaches with a variety of vulnerable clients and uphold the mission and values of SVdP.
3. OTHER DUTIES AND RESPONSIBILITIES -
- Maintain adherence to Company policies, safety/ergonomic standards and good housekeeping practices.
- Provide coverage as needed for other teams within the HES department.
- Perform other duties and responsibilities as required.
4. MINIMUM QUALIFICATIONS AND EXPERIENCE -
- Previous work experience in case management within a direct service or social service setting.
- An understanding of how trauma, systems, systemic oppression, race, ethnicity, gender, and sexuality impact people experiencing homelessness and poverty.
- Knowledge of the mental, emotional, and physical impacts of trauma.
- Experience maintaining client confidentiality and a professional code of ethics.
- Active listening skills and ability to acquire information in healthy ways and help problem solve sensitive issues non-judgmentally.
- Computer proficiency and ability to use smartphone, email, Microsoft Office Suite, and other programs necessary.
- Knowledge of the local social services system or system navigation.
- Skills to create and maintain healthy professional boundaries.
- Strong verbal, written, and interpersonal communication skills.
- Ability to understand complex written and oral instructions/rules/procedures.
- Ability to articulate program guidelines and requirements to those with a variety of abilities and backgrounds.
- Knowledge of the daily realities and stressors facing unhoused people.
- Possess a valid driver’s license and in good standing with SVdP’s driving policy.
- Bilingual is a plus.
