Summary
Provides assessments, individual recovery sessions and co-facilitates group sessions for clients in outpatient addiction treatment aimed at sobriety and/or harm reduction while improving client’s capacity to attain and sustain income and housing. Provides individual recovery sessions, group sessions, crisis intervention for clients in outpatient addiction treatment aimed at sobriety and/or harm reduction and improving client’s capacity to attain and sustain income and housing.
Essential Functions
- Conducts AOD Assessments, offers individual counseling, provides referrals and some case management, documents progress note and formulates Treatment Plans goals with clients. Provides individual recovery sessions to clients and completes accompanying session documentation in database. Completes timely clinical documentation and participates in weekly peer review and MAS meetings. Monitors client progress and modify their treatment plans as needed.
- Prepares curriculum materials & co-facilitates group treatment sessions for clients.
- Offers support through walk-in office hours, on-call support to providers and answers questions about program services.
- Supports other staff by assisting with referrals, performing outreach, attending Multidisciplinary Approach to Services, and discussing mutual clients.
- Coordinates with social and medical agencies, healthcare providers and community outreach programs to provide clients with needed services. Follow up on referrals from FJV and community.
- Provide MAT Supportive Counseling through same-day appointments and on-call supportive counseling for MAT clients/patients.
- Working towards hours for state certification and stay up-to-date on the latest practices.
- Attend weekly Supervision.
- Prepares curriculum materials & provides group treatment sessions for clients.
- Provides psycho-educational trainings, including NARCAN administration and Harm Reduction
- Complete continuing education courses to maintain Counselor certification and stay up-to-date on the latest practices.
- Train and support new interns.
- Ability to function independently while also working closely with multidisciplinary team to coordinate care
- Trains new interns and supervises entry-level counselors. May provide weekly or individual supervision to interns and uncertified counselors.
- On time completion of assigned training and policies.
- Performs other duties as assigned.
Qualifications
- Associates and/or Bachelor's Degree in counseling or related field.
- CPR certified (can be obtained after hire) where appropriate.
- 1 to 3 years of experience and Certified as Counselor.
- At least 1 year of experience working with clients experiencing homelessness, low income or at risk population, and Certified as Counselor
- Basic user of MS Office and electronic health record systems
- Good time management skills
- Superb verbal and written communication skills.
Participate in an annual Tuberculosis screening and/or other screenings when necessary. Reasonable accommodations may be granted
