Summary

This Case Manager II is specific to the Single Adult Transitional Housing Program (SATH) focusing on income and housing.  The Case Manager II is responsible for assisting the Supervisor in the leadership and functioning of their assigned caseload in this Interim Housing program for persons living with HIV/AIDS and their families.  Develops professional and empathetic relationships with clients while providing interventions related to housing through Case Management. Creates plans with clients toward achieving self-sufficiency. Develops and maintains a strong working relationship with housing providers to educate them about the benefits of making affordable housing available to homeless clients. Is an active participant in the client’s search for permanent housing, including searching for units, assisting with application processes, advocating to landlords, and assisting with all steps of the lease-up process.

Essential Functions

  • Manages a diverse caseload of up to 23 clients, and provides ongoing intensive support and guidance to clients through weekly case management meetings.
  • Creates an individualized care plan that includes coordinated services within the organization’s residential, social services, and health clinic including warm hand transfer to medical, substance recovery, behavioral services, and other resources within and outside the community to meet clients’ basic and extended needs, with emphasis on maintaining health outcomes. 
  • Provides ongoing intensive support to clients which can include assessing, evaluating, crisis intervention, applications for benefits, information, education, advocacy, and other supportive services. Completes 1 housing plan per month.
  • Assesses clients for employment, social security, and disability insurance eligibility; gathers records, assists with completing applications for benefits, provides referrals to community resources, schedules services/treatment with providers, and monitors progress.
  • Completes budgets and savings reviews; collects income verification, and calculates Shared Living Expenses (SLE) on a monthly basis.
  • Completes risk assessment and updates case plans every 90 days.
  • Prepares, presents, and timely documents client interactions and case plans; participates as a member of the multidisciplinary approach to services team that prepares and provides client case presentations.
  • Helps Supervisor in facilitating ongoing SATH events such as PRIDE, recovery, health fairs, and employment, Case Management Orientation
  • Utilizes a strength-based / trauma-informed approach to services, uses Motivational Interviewing techniques, and CREED to encourage positive change.
  • Maintains CSTAR client database, provides reports as needed, 
  •  Update ADP goals by monthly throughout the year
  • On-time completion of assigned training and policies.
  • Performs other duties as assigned.

Qualifications

  • Bachelor’s Degree in Social Services or related field, or equivalent experience, and one (1) year of applicable job experience, and
  • At least 12 months of experience working with underserved populations, case planning, and completing assessments in a social service setting.  
  • Knowledge of HIV/AIDS and those experiencing a SUD is beneficial.  
  • Bilingual (English and Spanish) is a plus.
  • Demonstrated basic understanding of principles of Housing First, Trauma Informed Care, Conflict Resolution, Motivational Interviewing, Low Barrier Operations and Prevention and Diversion practices, Housing First, social services, case management, eviction prevention, and crisis intervention. 
  • Knowledge of the housing market in San Diego and surrounding areas.
  • Ability to foster working and productive relationships with landlords, funding sources, and internal departments.
  • Basic user of MS Office. 
  • Must be able to pass a background check and fingerprint upon employment. 
  • Must have a valid California Driver’s License and be able to qualify for insurance coverage.
  • Participate in an annual Tuberculosis screening and/or other screenings when necessary.
