Summary:
The Integrated Care Coordinator provides patients with assessment, health education, insurance coverage information, and support in navigating/accessing integrated medical, dental, psychiatric, and behavioral health services within Father Joe’s Villages (FJV) Detox. This role works closely with Village Health Center (VHC) staff to assure patients receive timely, comprehensive care. The Integration Coordinator collaborates with VHC providers who require support with “warm hand-offs” between care services to assure high-quality care and patient stabilization. The Integration Coordinator will play a vital role in the patient intake process, ensuring individuals seeking detox services receive the appropriate support and resources.
Essential Functions:
- Provides care management and health care navigation services with a Detox Facility and in collaboration with adjacent Federally Qualified Health Center serving individuals and/or families experiencing homelessness and/or external agencies.
- Uses strong customer service skills to perform outreach and engagement in-person and via phone, assessment/evaluation of needs, provides education, meets and/or distributes materials to patients, coordinates and schedules services, and/or performs warm hand-offs to needed services.
- Serve as the first point of contact for prospective patients seeking detoxification services, providing them with information on available programs, admissions procedures, and the detox process including completion of necessary paperwork.
- Monitor intake schedules and collaborate with clinical and administrative teams to ensure timely and efficient patient intake.
- Maintain up-to-date knowledge of admission protocols, insurance policies, and regulatory requirements.
- Assist with insurance verification, authorizations, and benefit coordination to help patients understand their coverage and financial responsibilities.
- Prepare and maintain patient files, ensuring all documentation is accurate, confidential, and compliant with healthcare regulations.
- Participates as a member of a multidisciplinary team. Effectively communicates and collaborates with patients, other health professionals, clinical staff, and colleagues.
- Works to create a positive working relationships with VHC/FJV team members as well as community partners. Creates and maintains a contact list with phone, fax and email information.
- Participates in meetings, presentations, workgroups and/or learning collaboratives, as assigned.
- On time completion of assigned trainings and policies.
- Performs other duties as assigned.
Qualifications:
- High School Diploma or GED. Bachelors degree, preferred.
- Bilingual (Spanish/English) written and verbal, preferred.
- Previous experience with Drug Medi-Cal and/or managed care plans, preferred.
- RADT registration or equivalent (e.g., CAADAC, CADC) through an Alcohol and Other Drug certifying body required.
- Experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred.
- Ability to work independently and coordinate multiple tasks.
- Ability to interface with all levels of personnel in a professional manner.
- Minimum 1 year experience in a healthcare setting. Equivalent combination of education and experience that provides the skills, knowledge, and ability to perform the essential job duties.
- Experience with electronic health record systems.
