Your Role:
- MDS Coordination: Coordinate and complete MDS assessments and care plans for our residents.
- Accuracy & Compliance: Ensure the accuracy and completeness of MDS assessments and care plans, adhering to state and federal regulations.
- Team Collaboration: Work closely with the interdisciplinary team to develop and implement personalized care plans.
- Resident Progress: Monitor and evaluate resident progress, making adjustments to care plans as needed to ensure optimal outcomes.
Key Responsibilities:
- Assessment Coordination: Complete and coordinate MDS assessments and resident care plans.
- Quality Assurance: Ensure all MDS assessments and care plans are accurate and thorough.
- Collaborative Care: Develop and implement care plans in collaboration with the interdisciplinary team.
- Ongoing Monitoring: Track and assess resident progress, updating care plans as required.
- Regulatory Compliance: Maintain compliance with all relevant state and federal regulations.

Requirements
· Registered Nurse (RN) with a valid New Hampshire license
· RAC-CT certification preferred
· Minimum 2 years’ experience in long term care, including experience with the RAI process and person-centered care planning
· Must have strong clinical and communication skills
· Extensive knowledge of the RAI process and reimbursement requirements for both federal and state
