In this critical role, you will be responsible for providing direct and indirect social services to clients and their families to facilitate their adjustment to and integration into the community.
Key Responsibilities:
- Coordinate care amongst mental health, substance abuse, and medical providers
- Support clients diagnosed with serious mental illness (SMI) and those with chronic conditions
- Develop and implement personalized care plans to meet clients' unique needs
- Collaborate with healthcare providers, family members, and other stakeholders to ensure comprehensive care
Requirements:
- BSW or BA/BS in a related field (social work, psychology, etc.)
- One year of case management experience
- Valid New York State driver's license and reliable transportation
- Proficiency in Microsoft Office and software literacy
