Job Description
Under the direction of the department leadership, provides social work services consisting of discharge planning, outreach and follow-up, and advocacy. Provides case management services for assigned patients. In addition, the Discharge Planner is responsible for the education of patients, families and / or significant others. Provides community education as directed. Participates in related functions of the department.
Prior experiende in hospital or community health agenccy preferred; prior experience with assigned patiennt population may be required.
Additional language fluency may be preferred.
Computer literacy preferred.
Pay : 1199 Collective Bargaining Rates
