Saint Joseph's Medical Center is seeking a Health Facilitator to complete the registration and intake for the student visit, performs intake screening (weight, height, blood pressure) and other screenings as directed by the nurse practitioner. Provides minor care (first aid) within the scope of practice and refers to the nurse practitioner, school nurse or principal if indicated. Ability to work in a school setting (schools located in Yonkers).
Enter all required information for registration, scheduling visits as directed, track and follow-up on appointments, reach out to parents for missing information. Able to work with electronic health record and maintain accurate patient records. Ensures the registration forms, insurance verification and all required consents are on file.
Maintains a professional and courteous manner in all interactions with school offices, students, and SJMC staff.
Requirements:
Pediatric/child/adolescent experience in a school setting is preferred.
CPR certification.
Bilingual (Spanish/English) preferred.
Hourly rate: $23.0013
