Key Responsibilities :
- Provide direct nursing care to geriatric patients in their homes, following individualized care plans.
- Monitor and record patients' vital signs, symptoms, and overall health status.

- Assist with daily living activities, including bathing, dressing, and mobility support.
- Educate patients and their families on health management and disease prevention.
- Collaborate with healthcare professionals, including physicians, registered nurses, and therapists, to ensure comprehensive patient care.
- Maintain accurate and timely documentation of patient care and progress.
- Respond to emergencies and provide appropriate interventions as needed.
