Job Summary
We are seeking a Certified Alcohol and Substance Abuse Counselor (CASAC) or equivalent credential to join our dynamic team as a Case Manager . This role is responsible for coordinating care, advocating for clients, and ensuring a seamless transition through different levels of treatment. The ideal candidate has experience in substance use disorder treatment, strong clinical documentation skills, and a passion for helping individuals achieve long-term recovery.
Responsibilities
- Case Management & Client Advocacy :
- Develop, implement, and monitor individualized treatment plans.
- Act as the primary point of contact for clients, families, and referral sources.
- Collaborate with clinical staff to ensure continuity of care and treatment effectiveness.
- Assist clients in accessing resources such as housing, employment support, and aftercare programs.
- Clinical Documentation & Compliance :
- Maintain accurate and timely documentation in compliance with state, federal, and accreditation standards.
- Complete assessments, progress notes, and discharge planning reports.
- Ensure all treatment plans align with evidence-based practices and facility policies.
- Collaboration & Communication :
- Work closely with therapists, medical staff, and community organizations.
- Participate in multidisciplinary team meetings to review client progress.
- Facilitate psychoeducational and support groups as needed.
