Description:
This position oversees the operations of the community’s memory care program, aligning resident needs, state regulations, and Gardens on Quail Senior Living policies and procedures to provide exceptional care to residents.
Coordinator Job Description
- Assists the Director of Health and Wellness
- Oversee care team on floor- floor leader- 75% of time should be on floor.
- Communicate with families – eyes and ears for families.
- Final line of defense for schedule coverage- must have open availability.
- Coach, train, and support care staff
- Personal care plan building
- Family Engagement
- Maintains Resource Binder on each cart.
- Review Carestream tasks not completed daily.
- Review exception charted daily.
- Review PRN orders given/ensure efficacy daily.
- Remove discontinued medications from cart.
- Assist with faxing medications to Pharmacy and verifying in resolution que.
- Order medications and supplies
- Participate in review of initial referral/resident assessments- Nurse
- Admission medication review with resident/family day of admit.
- Participate in creation and updates to care plan.
- Incident review/investigation and education
- Care staff meetings
- Weekly med cart audits and review of Cycle fill each month.
- Weekly wander guard checks to ensure they are activating alarm.
- Daily check to ensure wander guards are attached to right ankle.
- Monthly audits for face sheet/MOST form, laundry decal, in case of fire decal, butterfly indicating hospice are in place.
- Assists in coordinating resident care and often stepping in to provide resident care.
- Participate in Care conferences.
- Controlled substance disposal and documentations
- Medication disposal and documentation
- Participate in yearly evaluations for Care staff.
- Quarterly Medication audits
- Participate in Quality Management Program
- Demonstrates a team approach, responsibility, accountability and professionalism.
Qualifications
- 1 year of experience as a coordinator
- 3 years of memory care experience
- Open Availability
- Multitasking, ability to lead a team and communicate to leadership above them.
- proactive and problem-solving abilities.
- Have or obtain state certifications and licenses.
- Ability to communicate with a culturally diverse care staff and residents.
- Model leadership, respect, & Maturity to team members, residents, and their families.
- Demonstrated experience supervising team members and providing training and support.
- Understand and follow State regulations as well company policies and guidelines.
- English language required.
- CPR Certification (preferred).
- Must pass criminal background check and drug test.

- 3 years of memory care experience
- Have or obtain state certifications and licenses.
