At Standard Healthcare Plus (SHCP), we are dedicated to helping individuals with severe mental health challenges, providing them with quality, effective treatment, so they can take control of their lives. We are a well-established mental health agency based in Clinton, MD, now expanding into Hagerstown and Frederick, MD to better serve our communities. We are seeking several dedicated and compassionate Case Workers to join our team. The ideal candidate will play a crucial role in supporting individuals and families in navigating social services and resources. This position involves working closely with clients to assess their needs, develop personalized treatment plans, and provide ongoing support to help them achieve their goals. The Case Worker will also collaborate with various agencies and community organizations to ensure comprehensive care.
Duties
- Conduct assessments to determine client needs and eligibility for services.
- Develop and implement individualized service plans based on client goals.
- Provide crisis intervention and management to clients in need of immediate support.
- Facilitate access to resources such as housing assistance, healthcare, and childcare services.
- Monitor client progress and adjust service plans as necessary.
- Advocate for clients within the social service system, including navigating Medicare and Section 8 housing programs.
- Collaborate with multidisciplinary teams, including social workers, healthcare providers, and community organizations.
- Maintain accurate documentation of client interactions and case progress.
Skills
- Strong knowledge of social work principles and practices, particularly in child welfare and behavioral health.
- Proficiency in crisis management techniques to effectively support clients in distress.
- Familiarity with addiction counseling methods to assist clients facing substance use challenges.
- Excellent communication skills, both verbal and written, for effective client interaction and documentation.
- Ability to work collaboratively with diverse populations and understand cultural sensitivities.
- Strong organizational skills to manage multiple cases simultaneously while ensuring quality care.
Education and Experience: Minimum of an Associate Degree or 3 years experience in behavioral health, case management, or psychiatric rehabilitation (Preferred).
