What are the responsibilities and job description for the Community Diabetes Technician-Diabetes Progra position at Standing Rock Sioux Tribe?
SCOPE OF WORK:
The Community Diabetes Technician will promote diabetes awareness of the benefits of living an active lifestyle to prevent diabetes and complications and will conduct outreach to provide support to individuals affected by diabetes.
SUPERVISION:
The Community Diabetes Technician will work under the supervision of the Fitness Project Manager, who will provide guidance, direction and conduct performance evaluations.
DUTIES AND RESPONSIBILITIES :
- Conduct outreach activities including but not limited to issuing referrals to at-clients to Indian Health Services for further evaluation.
- Provide notices and following up with clients regarding upcoming events.
- Assist with coordinating activities with the Diabetes Team, other programs, and schools with diabetes related education sessions and community events.
- Assist the Diabetes Educator and PHN's to ensure adequate care is being received by referred individuals with diabetes.
- Maintain records of client referrals, encounter forms, call log, client follow-up and submit timely monthly reports as required.
- Assist in data entry for community events and school screenings.
QUALIFICATIONS:
- High school diploma/GED with experience in health, nursing or other related field. A two (2) year degree in health, nursing or other related area is preferred.
- Must have the ability to communicate with the public.
JOB REQUIREMENTS:
- Required to adhere to SRST Tribal Personnel and Financial Policies and Procedures and SRDP Fitness Center policy and procedure manual.
- Adhere to confidentiality requirements; breaking of confidentiality will be grounds for dismissal.
- Attend training and certifications CPR/First Aid, and other professional development trainings as required.
- Attend diabetes team meetings as scheduled.
- Ability to work with the public, colleagues and others; conduct self in a professional manner.
