QUALIFICATIONS
Education & Experience
- Community Resource Navigator I:
- High School Diploma or equivalent AND 2 years’ experience providing community engagement, community resources/linkage, or direct service support to youth & families
- AA Degree in social service, psychology, juvenile justice, sociology, child development or health/human services related field AND 1 years’ experience providing community engagement, community resources/linkage, or direct service support to youth & families
- Community Resource Navigator II:
- Bachelor’s degree in social service, psychology, juvenile justice, sociology, child development or health/human services related field AND 2 year experience providing community engagement, community resources/linkage, or direct service support to youth & families
- Master’s degree in social service, psychology, juvenile justice, sociology, child development or health/human services related field AND 1 year experience providing community engagement, community resources/linkage, or direct service support to youth & families
Bilingual skill preferred but not required
ESSENTIAL JOB FUNCTIONS
Position Specific:
- Identifies underserved populations and community locations to educate the public and identify youth and caregivers who may be eligible for agency continuum services and those eligible for Medi-Cal specialty mental health services (SMHS)
- Conducts culturally and linguistically appropriate community engagement & community training focused on engaging and linking youth & families to agency continuum of services & increasing unlinked Medi-Cal youth and families to services & Medi-Cal benefits.
- Creates and compiles materials to inform youth and families about the agency continuum of services and the broader Medi-Cal program & benefits and the steps to enroll or re-enroll.
- Engage and support engagement at various community locations but not limited to resource centers, homeless shelters, schools and other locations to increase community awareness and linkage of youth & families to the agency Medi-Cal and/or Medi-Cal covered services.
- Distribute flyers, and educate at risk and underserved populations, such as women in need of family planning and pre-natal care, individuals experiencing homelessness, mental health and substance abuse disorders
- Develop strategic partnerships with local community organizations, resource centers, schools and medical facilities to increase youth & family linkage to agency continuum of services and linkage to Medi-Cal and the appropriate Medi-Cal services.
- Provides follow-up support & contact to ensure youth & families are appropriately linked and connected to the agency continuum of services and/or Medi-Cal services.
- Assist eligible youth and their caregivers in applying for Medi-Cal, including attending Medi-Cal intake appointments, assisting the client in understanding and completing the Medi-Cal application process
- Assist eligible youth with providing third party resource information at Medi-Cal eligibility intake appointment and in providing and packaging forms in preparation for Medi-Cal eligibility determination
- Assist eligible youth with the Medi-Cal appeals processes, including the Medi-Cal administrative hearing process
- Possess and demonstrate specific cultural competence to work with and outreach to culturally diverse populations and communities
- Work collaboratively with all internal programs, departments, and staff to coordinate care and provide support as needed
- Maintain positive relationships with community partners, families and providers
- Provides program service provision through enhance care management and mental health programs through intakes, care coordination and therapeutic support as needed

- Works collaboratively with all agency programs and staff to provide support as needed
- Utilizes and maintains calendar with all work-related details to manage time effectively and share calendar information with coworkers
- Participates in on-going training to expand and develop professional skills
- Performs other duties as necessary for the agency, as assigned
Employment At-Will
Employment at the Agency is terminable at-will, which means that employment may be
terminated at any time, without cause or reason, by either the employee or the Agency. In
addition the Agency may also demote, layoff, transfer or reassign employees at any time at its
sole discretion without cause or reason.

Starting Salary Range:
Community Resource Navigator I: $25.00 - $30.00 hourly DOE
Community Resource Navigator II: $68,640 - $72,707 annually DOE
