REQUIREMENTS : Current Resume
LPN- PA license (in good standing)
Sub-Acute / SNF / Long-Term Care experience (1 year)
Current - BLS / CPR
2- Current references (RN- managers)
2- Step PPD (within the last 12 months)
Current physical (within the last 12 months)
Vaccination record : MMR, Varicella, Hep B
Must be able to pass criminal clearance check / back ground check
Must be able to pass urine drug screen
