Facility: Skilled Nursing
Location: Eureka, CA
Start Date: ASAP
Shifts: X2 for PM shift (8hr. 4on/2off)
Length: 4, 6, 8, or 13-week contract
Responsibilities and Duties
- Provides for activities of daily living by assisting with serving meals, feeding patients as necessary, ambulating, turning, and positioning patients; and providing fresh water and nourishment between meals.
- Maintains patient stability by checking vital signs and weight, testing urine, and recording intake and output information.
- Provides patient comfort by utilizing resources and materials; transporting patients; answering patients’ call lights and requests; and reporting observations of the patient to the nursing supervisor.
- Documents actions by completing forms, reports, logs, and records.
- Maintains work operations by following the policies and procedures of the facility
- Protects the organization’s value by keeping patient information confidential.
- Enhances nursing department and facility reputation by accepting ownership for accomplishing new and different requests; and exploring opportunities to add value to job accomplishments.
- Checks residents/patients frequently to ensure that their personal care needs are being met.
- Treats all residents, visitors, and staff with courtesy
- Maintaining professionalism at all times

- Nursing license – LVN (CA)
- Current BLS/CPR

- Physical (within the last 12 months)
- PPD (within the last 12 months)
- MMR, Varicella, Hep B (or decline)
- Tdap/Flu (or decline)
