StarCare Specialty Health System (StarCare) is seeking a qualified and compassionate individual to work as a full time Care Coordinator for outpatient mental health services at Sunrise Canyon Hospital.
Responsibilities:
- Care Coordination works with consumers and interdisciplinary team in order to assist in development, management and maintenance of the consumer’s healthcare plan.
- Ensure care coordination is implemented by managing client caseloads, facilitating conversations between interdisciplinary Care Team members including Care Coordinators, primary care physicians, and additional providers.
- Organize schedules and managing appointments for patients to ensure they receive services in a timely manner
- Facilitate communication between the patient, family members, and healthcare providers to ensure patient satisfaction and quality of care.
- Assist in identification and application for appropriate benefits.
- Manage follow up visits and ensuring continuation of care and linkage to community resources.
- Ensure that Center resources are utilized appropriately.
Essential Duties:
- Coordinate with Case Management and Clinic Staff to ensure consumers are being seen in a timely manner and are aware of upcoming appointments.
- Manage lobby flow
- Maintain Outpatient clinic schedule
- Complete and submits documentation timely and professionally.
- Assist the supervisor or other Center Staff with special projects or assignments following appropriate guidelines in order to insure the efficient operation of the Center.
Schedule: This is a full-time/non-exempt position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

Qualifications
Qualifications:
- Graduation from an accredited four (4) year college or university with major course work in a related field required.
Skills & Abilities:
- Ability to foster collaborative relationships with key workforce members.
- Exhibit excellent oral and written communication with the ability to communicate to all levels of the organization.
- Exhibit excellent interpersonal skills with a team oriented work style.
- Organized, detail oriented, computer proficient, and have the ability to write clearly and concisely.
- Ability to respond to individuals with patience, empathy, and understanding.
- Experience in working with individuals with serious mental illness, substance use disorders, and/or severe emotional disturbances.
- Knowledge of the social services system and the ability to guide workforce members in connecting individuals with appropriate services.
- Ability to work under general supervision with moderate latitude for the use of initiative and independent judgment.
Additional Requirements:
- Must pass a pre-hire drug screen and criminal background check.
- Must have a Texas driver's license and be insurable under the Agency's insurance.
