This role involves collaborating with hospital staff on discharge planning, connecting TAY youth to community social services, and coordinating in-person visits with contracted providers. Additionally, the ECM advocates for TAY youth with healthcare professionals monitors treatment adherence (including medication), and employs a "whatever it takes" approach to providing services across various settings such as homes, schools, and community locations. Responsibilities also include transporting participants, accompanying them to office visits, providing crisis intervention, and facilitating case management and linkage services. All services are documented in the Electronic Health Record, with all the required outcome measures completed.
