SUMMARY:

Responsible for ensuring continuity of care through advocating, planning, linking, coordinating, and monitoring of services and resources to help improve the quality of life for individuals served.

Pay :19.25/hr

Successful completion of Background, MVR and Drug Free Workplace Screenings.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Electronically documents case management activities according to Agency, State, Medicaid and other accreditation standards and policy and procedures.
  • Provides program linkages with other agency programs and community resources to coordinate service delivery, remove impediments to effective care, and achieve established objectives.
  • Advocates for and links the individual with identified needed services and resources. Coordinates all services ensuring individual's needs are being met toward completion of service plan goals.
  • Works with the individual's family and natural support system in the individual's natural environment.
  • Monitors and ensures compliance in service delivery and documentation.
  • Participates as a team member and contributes to the enhancement of the department and agency.

MENTAL HEALTH COURT CASE MANAGER SPECIALITY/POPULATION SPECIFIC DUTIES:

  • Have knowledge and understanding of the Mental Health Court program, handbook, and phases.
  • Review and monitor all Mental Health Court participant's appointments with their medical providers, appointments with their therapists, weekly groups, and drug screenings.
  • Coordinate with the Mental Health Court team including the medical provider, therapists, peer, court case managers, attorneys, and the judge.
  • Facilitate the weekly Mental Health Court group module.
  • Complete weekly court reports and input them into the DCCM website.
  • Attend bi-weekly Mental Health Court hearings.
  • Participate in the weekly SPBH MHC staffing.
  • Complete medication counts and Urinary Drug Screenings as needed.

EDUCATION and/or EXPERIENCE:

Bachelor's degree from an accredited four-year college or university with emphasis in the areas of psychology, social work, health education or related human services field. Preferred one year of full time or equivalent experience working with adults who have serious mental illness and children who have serious emotional disturbances.

CERTIFICATES, LICENSES, REGISTRATIONS:
Certification or certification-eligible by Medicaid as a Case Manager

SUPERVISORY RESPONSIBILITIES:

There are no supervisory responsibilities for this position.
