Overview:
The Transitional Care Tele Triage LPN provides services to the assigned population in accordance with protocols. The transitional primary care LPN serves as a resource for telehealth patients and providers. You will serve as a coordinator for this virtual visit program.
Responsibilities:
- Schedule needed virtual visits in coordination with telehealth provider
- Receive and upload records into HCHB, starting client chart
- Send RingCentral link to designated HH clinician and client for scheduled visit
- Ensure compliance with state/local requirements through proper documentation
- Participate in performance improvement activities and maintain ongoing clinical knowledge
- Document in real time
- Actively participate in applicable meetings as requested
- May complete SNF Triage LPN duties as needed
- 2 years of experience in a clinical healthcare setting
- High school diploma/ GED required; Associate’s Degree or some college preferred
- Licensed Practical Nurse (LPN) with an active license; compact license preferred.
- Experience with electronic medical record software and proficient use of the Microsoft Office suite
- Ability to multitask, prioritize, and manage multiple projects simultaneously
- Excellent written and verbal communication skills and ability to work independently or in a team setting
