About the Facility:
Grand Junction Regional Center (GJRC) is one of three Regional Centers in Colorado that serve people with intellectual and developmental disabilities who have intensive needs.
GJRC, which dates back to 1919, has both Intermediate Care Facilities/Intellectual Developmental Disabilities (ICF/IDD) homes and Home and Community Based Services Waiver (HCBS Waiver) homes throughout the community. The center serves a diverse population, from people who have high abilities but are a high risk to themselves or others to people who have significant medical challenges.
Programs are individualized to meet varying needs. Services provided range from residential services and health services to mental health intervention. GJRC provides day program, vocational and specialized habilitation, speech, occupational therapy, therapeutic recreation and nutritional services.

Description of Job
This position is essential in the coordination of Community Integration and Habilitation Services (CIHS) for disabled people living at Grand Junction Regional Center (GJRC).
Additionally, as a CIHS Supervisor, you can expect to:
- Develop and coordinate appropriate CIHS supports and services for people in ICF and HCBS Waiver engaging in activities of daily living, community connections, supported employment and specialized habilitation.
- Monitor documentation/billing for reimbursement. Ensure compliance with federal regulations, state regulations, division policies and agency procedures.
- Exercise supervision over the CIHS employees
- Complete Performance Management Plans- Core competencies, goals and professional development
- Oversee timekeeping and leave approval via Kronos/UKG, W2W and Google Calendar.
- Plan for certification/funding audits and allocates resources to meet annual and daily needs of CIHS staff, sites and work locations. This includes, but is not limited to budget, staff scheduling, annual services for people served, transportation and supplies.
- Identify, evaluate, and ensure compliance to CDHS policies, DRC policies and agency procedures/protocols.
- Collect, complete, monitor, and delegate administrative forms, reports and records in order to achieve ongoing licensure and certification.
- Complete duties using prescribed electronic and hard copy procedures utilizing computers and other supporting tools as needed to complete the duties.
- Participate in agency work groups and committees; attends assigned meetings and in-services as scheduled.
- Train HCT III staff in departmental process, implementation of CIHS supports, assessment completion, quarterly/annual meeting documentation, and ISP program writing/monitoring.
- Annual recertification is required for “CORE”, delegated skills, and Safety Care.
Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights
Minimum Qualifications:
High school diploma or General Education Diploma (GED).
AND
Three (3) years of experience in an occupational field related to the work assigned to the position.
Substitutions:
Additional appropriate education will substitute for the required experience on a year-for-year basis.
Special Qualifications:
SBON Psych Tech license (LPT) or Qualified Medication Administration Person (QMAP) required for this position; valid, current and unrestricted.
Preferred Qualifications:
- Experience training, mentoring, and/or supervising healthcare staff
- Experience conducting and writing assessments
- Experience working with individuals with intellectual and developmental disabilities or dully diagnosed
- Experience working with individuals with compromised medical history
- Experience working with individuals with challenging behaviors
- Experience with Word, Excel, Google Calendar, Google Sheets
Conditions of Employment:
- Preemployment physical Test: must demonstrate the ability to lift 50 lbs, bend, kneel, squat, etc. and perform physical Interventions proficiently throughout employment.
- CDHS employees (all Direct contact with vulnerable persons): CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, Trails check (direct contact with children), CAPS
- Current/Valid and unrestricted US Driver’s License
- Shift Work - GJRC is a 24/7 facility; therefore, person in this position must be available to work any shift, any day, and/or any area, to meet agency needs.
- Must be 18 years or older.
- Travel - Position must travel to remote work sites and must utilize a State vehicle, if available. If not, must utilize personal vehicle and submit for mileage reimbursement. Position is required to use a state vehicle to transport residents.
- Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
