Job duties include, but are not limited to:
- Provide direct resident care including bathing, dressing, undressing, assisting in and out of bed, performing personal hygiene, feeding programs, toileting programs, range of motion, transporting, and interacting socially to meet residents’ basic needs.
- Explaining patient care before, during and after contacts
- Answering call lights and assessing basic resident care needs
- Updating noted resident changes, reporting information to the charge nurse and recommending routines and schedules to IDT Team
- Performing basic resident observations, such as vital signs, blood pressure, weight, intake and output, and basic skin assessments
- Providing infection control by implementing/using universal precautions and proper disposal of waste
- Cleaning linen room and equipment, stocking supplies, and completing supply forms
- Correctly using safety/positioning devices, side rails, transfer belts, and other equipment,
- Attending on-going staff education programs.
One year of experience in an occupational field related to the work assigned to the position.
Note: This classification typically also requires a high school diploma or GED, but the CNA certification acts as a substitute for this education requirement.
- Current, valid licensure as a Practical Nurse (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact will substitute for the experience requirement.
Preferred Qualifications:
- Experience in a long-term care environment
- Experience working with veterans or patients who suffer from PTSD
- Experience working with patients with dementia
- Excellent communication and customer service skills
- Must be willing to submit to a full background check
- Must be willing to obtain vaccinations, as required by the State and facility
- Must be willing to test for Tuberculosis (TB)
- Must possess a valid, unrestricted U.S. driver's license
- May be required to travel/transport residents in state vehicles
- Required to have a current, valid CPR card
- Shift Work – While every effort will be made for the schedule to reflect the shift/days agreed upon by my supervisor and me, I understand I may be scheduled on different days/shifts based on facility need. While leadership will ask for overtime volunteers as a first-line strategy to relieve staffing shortages, I understand that I may be required to work overtime as a part of this position. Shift work includes weekends, nights, holidays.
- May require on-call status, if needed, per the pay plan for this position
- Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
