THE POSITION
The State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is now hiring for a Clinical Social Worker to join their team! This position will carry a caseload of 10-11 patients.

  • SCHEDULE: Full-Time, 40 Hours per Week, First Shift, 8:00am - 4:30pm, Monday through Friday

DISCOVER THE OPPORTUNITY TO
  • Participate in all unit admissions and discharges into community-based settings;
  • Collaborate and coordinate with inpatient and outpatient clinical teams, community affiliates, and other providers, conservators, and families for patient care;
  • Complete all required admission and discharge paperwork, conducts clinical assessment and evaluations, progress notes, documentation, and reassessments per agency policies;
  • Maintain and monitor compliance with inpatient unit’s environment of care, and Joint Commission/CMS/DPH health and safety standards;
  • Promote cultural and gender diversity, equity and inclusion in the workplace, sensitivity to client needs, and assures the maintenance of client rights
  • Understand Motivational Interviewing practices, Trauma Sensitive Principles/Practices as well as principles of Person-Centered Planning. Develops Individual Recovery Plans in collaboration with the interdisciplinary clinical team and patients
  • Provide therapeutic and recovery-oriented skills building to client in the program and provide crisis intervention whenever needed;
  • Make referrals and collaborates with community housing programs, mental health, substance use, medical, entitlement, legal and other providers and assists with linkage and engagement with needed resources;
  • Participates in case conferences, All Treater’s Meetings, and all meetings, trainings, conferences pertaining to clinical role;
  • Provide family psychoeducational interventions and other recovery oriented clinical groups and individual therapy as needed;
  • May provide direct transport and escort of clients to appointments and other community functions;
  • Maintains client’s rights to confidentiality, and accurately maintains records in the DMHAS Electronic Medical Record per DMHAS/SWCMHS policy and procedure.

SWCMHS strives to provide high quality, compassionate and culturally competent mental health and substance abuse services to adults with behavioral health disorders who are indigent. With three locations throughout Fairfield County, SWCMHS, promotes and administers comprehensive recovery oriented services in the areas of mental health treatment and substance abuse prevention and treatment. These services include inpatient treatment programs and outpatient clinical services, as well as a wide range of additional supportive treatment services.

PURPOSE OF JOB CLASS (NATURE OF WORK)

In a state agency behavioral health program as part of a multi-disciplinary team at one or more locations this class is accountable for independently providing clinical social work services.

OFFICE OF HEALTHCARE ADVOCATE: This class is accountable for serving as a case manager and advocate for clients that have received or need to receive behavioral health care services.

EXAMPLES OF DUTIES

  • As a member of multi-disciplinary clinical team or within a network of community-based behavioral health services develops psychosocial assessments through clinical interviews with assigned clients and families which include consideration of developmental, social and/or cultural, substance abuse, psychological, environmental and medical issues;
  • Develops treatment plans including measurable treatment objectives;
  • Provides clinical treatment services in the form of triage, crisis intervention, individual, group and/or family therapy, screening and planning;
  • Advocates and develops networks of social and clinical services to assist clients in meeting identified needs;
  • Monitors and evaluates effectiveness of treatment plans and documents treatment in clinical records;
  • Completes reports in accordance with agency and accrediting bodies;
  • Provides clinical supervision to Licensed Master Social Workers;
  • Consults and collaborates with other behavioral health providers to improve delivery of services to clients;
  • Provides forensic evaluations and court testimony as required;
  • Maintains social work treatment and standards as required;
  • May participate in research projects;
  • Performs related duties as required.

OFFICE OF HEALTHCARE ADVOCATE

  • Manages a caseload of consumer behavioral healthcare insurance complaints concerning coverage and/or payment;
  • Reviews and assesses medical records including psychological assessment and treatment plans, concerning but not limited to mental, emotional, behavioral and addictive disorders to determine appropriate treatment plans and eligibility for insurance benefit payment;
  • Participates in insurance appeal hearings and serves as clinical expert to defend and demonstrate that the diagnostic assessment and treatment plans are appropriate and required to address client behavioral healthcare needs;
  • Performs related duties as required.

KNOWLEDGE, SKILL AND ABILITY

  • Knowledge of
    • social work methodology, casework, group work and community mobilization;
    • family and interpersonal relationship dynamics;
    • values, sanctions, purposes and ethics of professional social work;
    • social, cultural, economic, medical, psychological and legal issues which influence attitudes and behaviors of clients and families;
    • mental illnesses and approaches to treatment;
  • Considerable

    • devise and implement a treatment plan with measurable goals that address client needs;
    • independently apply current psychiatric treatment modalities to address client needs.

MINIMUM QUALIFICATIONS

Licensure as a clinical social worker in the State of Connecticut.

PREFERRED QUALIFICATIONS

  • Experience providing inpatient clinical social work job functions (individual and group psychotherapy skill building interventions, and case management) in an intensive Behavioral Health Psychiatric Unit in collaboration with an interdisciplinary team of professional and paraprofessional staff
  • Experience coordinating Residential Program referrals and discharges into community-based settings and collaborating with inpatient and outpatient clinical teams and community affiliates
  • Experience providing intensive clinical social work services to individuals who are discharging from long term state inpatient hospitalization into community residential settings who have difficulty with severe chronic mental illness, substance use disorders, complex medical comorbidities, challenged immigration status, illiteracy, and hx of legal involvement
  • Experience making referrals and collaborating with community housing services, mental health, substance use, medical, entitlement, legal and other providers across the region’s social service system
  • Experience with crisis intervention
  • Experience using email, Microsoft Office software, and other computer software applications

SPECIAL REQUIREMENTS

  • Incumbents in this class must possess and retain a license to practice clinical social work pursuant to Sec. 20-195 (m) through (r) of the Connecticut General Statutes.
  • Incumbents in this class must possess and maintain eligibility for participation in federal health care programs as defined in 42 U.S. § 1320a-7b(f).
  • Incumbents in this class may be required to have knowledge of sign language and communication ability with the deaf and hearing impaired in certain designated positions.
  • Incumbents in this class may be required to have ability in Spanish oral and written communication in certain designated positions.
  • Incumbents in this class may be required to travel.
