This position independently applies the nursing process to individuals, ensuring the delivery of high-quality care by carrying out physician orders related to diet, medication, and treatment.   • Work Schedule: Full time, second shift | 4:00 P.M. - 12:30 A.M., every other weekend and holidays as scheduled

KEY RESPONSIBILITIES INCLUDE:
  • Conducting assessments and monitoring the physical and mental health status of individuals
  • Changing dressings and documenting nursing interventions in medical records
  • Ensuring the secure distribution and record-keeping of controlled substances
  • Maintaining and updating patient records, making routine reports
  • Making decisions for routine care situations, providing guidance to staff, individuals, and families about treatments, care plans, and health/mental health concerns
  • Responding promptly to emergencies and ensuring the availability of medications and supplies
  • Maintaining cleanliness and proper functionality of equipment and the environment
  • Collaborating as part of an interdisciplinary team to assess, develop, implement, and evaluate care plans
  • Formulating and implementing individualized nursing care plans
  • Participating in outpatient clinics, assisting with physical exams, and administering routine tests
  • Conducting staff training and delegating nursing tasks in accordance with protocols
  • Engaging in psychotherapeutic and rehabilitation interventions, including providing physical restraint when necessary
  • Providing specialized medical or mental health services as required
  • Serving as a Qualified Intellectual Disability Professional (QIDP), and performing other related duties as needed.

FOR THOSE WHO HOLD AN RN LICENSE IN A STATE OTHER THAN CONNECTICUT AND WISH TO APPLY FOR A CONNECTICUT LICENSE, PLEASE:

  • Review the eligibility and documentation requirements for CT license by endorsement of an out-of-state license;
  • Complete a CT DPH online application online (expand the ‘Public Health Practitioners’ grouping, select ‘Registered Nurse’ and then ‘Start’;
  • Request verification of your out-of-state license(s) electronically through the National Council of State Boards of Nursing’s : Nursys® System. To send verification of your license(s) to CT, read and agree to the terms, enter your biographic information, select your license type and select ‘SEARCH’. Review the results and select the ‘Next’ button. Check the box next to ‘Connecticut’ and complete the transaction. Once the transaction is completed, you will receive a confirmation email from Nursys and the verification will be available to the DPH. Note that PA and MI verifications are not provided through Nursys. Please contact those state boards for information on obtaining verification.

PURPOSE OF JOB CLASS (NATURE OF WORK)

In a state agency this class is accountable for providing a full range of professional nursing care.

EXAMPLES OF DUTIES

  • Independently implements the nursing process with individuals including carrying out physician's orders regarding diet, medication and treatment;
  • Performs assessments;
  • Changes dressings;
  • Monitors and ensures quality care through observation and evaluation;
  • Reports on physiological and mental health status of individuals;

  • May participate as a member of an interdisciplinary treatment team in the assessment, development, implementation and evaluation of individual's treatment or health/mental health care plans;

  • May delegate nursing tasks consistent with established protocols;

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE

Graduation from an accredited nursing program.

PREFERRED QUALIFICATIONS

  • Experience working in the community with clients diagnosed with severe and persistent mental illness and substance use disorders.
  • Experience working with a multidisciplinary team.

SPECIAL REQUIREMENTS

  • Incumbents in this class must possess and retain a license or temporary permit as a Registered Professional Nurse in Connecticut.
  • Incumbents in this class may be required to be fluent in a foreign language or proficient in the use of the American Sign Language in certain designated positions.
  • Incumbents in this class may be required to travel.

WORKING CONDITIONS

Incumbents in this class may be required to lift and restrain individuals; may have exposure to communicable/infectious diseases and risk of injury from assaultive/abusive individuals; may be exposed to disagreeable conditions.
