Your specific responsibilities
- Serves as Behavioral Health Referral Coordinator for the Health Care for the Homeless Program with responsibility to:
- Arrange referrals for behavioral health care services.
- Ensure timely and accurate collection and input of patient data.
- Monitor patient referrals for behavioral health care services and follow up on services that are overdue as appropriate.
- Provide behavioral health coordination services to referred patients in a timely manner by submitting referrals on a daily basis to the Provider.
- Coordinate patient follow up appointments as needed.
- Understand patient behavioral health care needs and effectively address those needs.
- Foster effective community partnerships to meet the behavioral health care needs of the patients.
- Serve as point of contact for behavioral health care partners.
- Copy, scan, and fax medical records and other documents.
- Maintain confidentiality of all records, patient interactions, and services according to HIPAA privacy policies
- Serves as backup to the Medical Referral Coordinator, with responsibility to:
- Print records from HMS and scan into NextGen
- Submit referral requests to Specialty Care department for authorization or extension
- Contact patients when referral is authorized or denied
- Perform other related duties as assigned including emergency duty
Minimum-
- Access to reliable transportation
- Experience working in a medical clinic environment, including knowledge of medical terminology
- Three or more years of customer service experience
- One of more years of behavioral health experience
Preferred-
- Three or more years of data entry experience
- Experience working with the homeless population
